Could this only be done in Adobe Photoshop? Or is there another program I could download (that is free) that I could use?

GIMP. Kind of complicated to learn, but extremely powerful (will do just about anything Photoshop will do). Also, loads of help sites and advice sites for it. Using GIMP, you can certainly produce a graphic superior to the example that you provided.
